The Personal Health Inventory is a guided self-assessment developed by the VA Whole Health program. It helps you evaluate eight key areas of your life and identify where you want to focus your health efforts.
We share this tool because it reflects our “Test, Don’t Guess” philosophy: understanding where you are is the first step toward meaningful change. This fillable document provides a structured starting point for your wellness journey.
Before you can make meaningful changes to your health, you need to understand where you are right now. The Personal Health Inventory (PHI) is a structured self-assessment that guides you through eight areas of the “Circle of Health”: working your body, food and drink, recharge, surroundings, personal development, spirit and soul, relationships, and the power of the mind.
At Nepute Wellness, our approach begins with assessment. Whether it is advanced lab testing, DNA analysis, or a simple self-inventory like this one, we believe that “Test, Don’t Guess” applies at every level. The PHI is a practical starting point for anyone who wants to take an honest look at their overall wellbeing before seeking more targeted support.
The PHI asks you to rate each area of health on a scale, reflect on what matters most to you, and identify one or two areas where you would like to focus. This is not a diagnostic tool; it is a self awareness exercise that complements professional assessment.
Research published in Global Advances in Health and Medicine has shown that this kind of structured self-reflection may help individuals take more proactive steps toward their health goals and feel more engaged in their care.
