The Personal Health Inventory is a guided self-assessment developed by the VA Whole Health program. It helps you evaluate eight key areas of your life and identify where you want to focus your health efforts.
We share this tool because it reflects our “Test, Don’t Guess” philosophy: understanding where you are is the first step toward meaningful change. This fillable document provides a structured starting point for your wellness journey.
Before you can make meaningful changes to your health, you need to understand where you are right now. The Personal Health Inventory (PHI) is a structured self-assessment that guides you through eight areas of the “Circle of Health”: working your body, food and drink, recharge, surroundings, personal development, spirit and soul, relationships, and the power of the mind.
At Nepute Wellness, our approach begins with assessment. Whether it is advanced lab testing, DNA analysis, or a simple self-inventory like this one, we believe that “Test, Don’t Guess” applies at every level. The PHI is a practical starting point for anyone who wants to take an honest look at their overall wellbeing before seeking more targeted support.
The PHI asks you to rate each area of health on a scale, reflect on what matters most to you, and identify one or two areas where you would like to focus. This is not a diagnostic tool; it is a self awareness exercise that complements professional assessment.
Research published in Global Advances in Health and Medicine has shown that this kind of structured self-reflection may help individuals take more proactive steps toward their health goals and feel more engaged in their care.
Download the fillable Personal Health Inventory below. Take 15 to 20 minutes to work through it honestly. Use your results as a conversation starter, whether with your Nepute Wellness practitioner, your primary care provider, or simply with yourself.

Selfcare as Healthcare: the first step is knowing where you stand.
Brain mapping is an advanced diagnostic tool used at Nepute Wellness to visualize how your brain is functioning. It helps our team identify areas of neurological imbalance, communication breakdowns between brain regions, and patterns that may be contributing to symptoms like anxiety, depression, chronic pain, or cognitive difficulties.
During your Day Two Report of Findings, your brain mapping results are reviewed line by line alongside your blood work, giving you a clear, recorded “map” of where your health stands and what steps are needed to help restore balance.
“Selfcare as Healthcare” is one of the guiding mantras at Nepute Wellness. It represents the belief that true healthcare starts with empowering you to take an active role in your own healing and your family’s health, rather than passively depending on the traditional sick-care system.
At Nepute Wellness, this means educating you about root causes, providing tools and resources for ongoing wellness, and giving you the knowledge to make informed decisions about your health journey. Our goal is to move you from confusion and overwhelm to certainty and empowerment.
Yes. During your Day Two Report of Findings, your provider presents a personalized Health Investment Plan that includes care timeline, recommended treatments, and associated costs. We work with patients to find arrangements that make comprehensive care accessible.
Unlike the traditional insurance model where costs are hidden and treatments limited, our transparent approach means you understand exactly what you’re investing in and why. Contact our team to discuss options that work for your situation.
